Torrance, CA, October 21, 2014 -- /EPR NETWORK/ -- HealthCare Partners announced today it signed a provider agreement with Health Net Community Solutions, Inc., a subsidiary of Health Net, Inc., to care for Cal MediConnect patients in Los Angeles County. Health Net is one of five health plans in Los Angeles County participating in the state’s three-year demonstration to promote coordinated health care delivery to those eligible for both Medicare and Medi-Cal.

“HealthCare Partners looks forward to bringing the benefits of our nationally recognized coordinated care model to this dual eligible population unaccustomed to having access to care under a single health plan,” said HealthCare Partners Chief Medical Officer Dinesh Kumar, M.D. “We are very pleased to work with Health Net to improve patient outcomes while lowering healthcare costs.”

HealthCare Partners will manage all medical care for patients in Cal MediConnect who select HealthCare Partners as their medical group, while also working with Health Net to help ensure patients receive the necessary long-term services and supports that may help them function independently in their homes and communities. Both organizations also bring extensive resources to the relationship to fully coordinate the behavioral health needs of this population.

“Health Net has long supported those in need of highly integrated care,” said Martha Smith, Health Net, Inc. chief dual program officer. “We are proud to work with HealthCare Partners to offer the necessary resources to meet the unique needs of Cal MediConnect beneficiaries.”

The agreement is pending Department of Managed Health Care (DMHC) approval.
